We sometimes forget as leaders that blame and accountability are not the same. When we blame, we are focused on pointing the finger and seeking retribution.
When we hold people accountable, we are setting expectations around responsibility and seeking engagement.

I particularly like the emphasis on creating a no blame culture. If people are worried about punishment or criticism, they are unlikely to speak up.
